1. You can place them in any order. But the arrangement must be field-marked as to polarity, grounding system, and nominal voltage [408.3(E)(2)].

2. It must be legibly and permanently marked with a caution sign that states how many volts the high leg (identifed by its phase number) has to ground [408.3(F)(1)].

3. Ungrounded AC system, high-impedance grounded neutral AC system, ungrounded DC system, and resistively grounded DC system [408.3(F)(2), (3), (4), and (5)].
   

4. Every circuit and circuit modification shall be legibly identified as to its clear, evident, and specific purpose or use[408.4(A)].
   
5. That depends on whether they (and their supports and other obstructions) are insulated. If so, then 8 inches. If not, then 10 inches[408.5 and Table 408.5].
